The controller is HealthUp Sp. z o.o. based in Warsaw (00-105), Twarda 18 Street. I have the right to withdraw my consent at any time (you can send information to the email address: firstname.lastname@example.org ). I have the right to access personal data, rectify, delete or limit their processing, the right to objection, the right to transfer data, the right to lodge a complaint with a supervisory authority.
The legal basis for the processing of personal data is art. 6 sec. 1 lit. a) the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and in the free movement of such data and repealing Directive 95/46 / EC (GDPR). The data will be processed until withdrawal of consent (original purpose). However, after withdrawal of consent, the processing is continued for for the establishment, exercise or defence of legal claims (secondary purpose). Processing for secondary purposes is limited only to the storage of information about consent and its withdrawal for a time specified by law in the scope of pursuing claims.
The controller uses various IT solutions that allow for more efficient communication and cooperates with entities supporting it in business and IT processes (so-called data processors) and they are recipients of data. Data is not transferred outside the European Economic Area.
